Caremerge and Lively announce partnership to improve senior care

Chicago, IL, February 5, 2014 – Caremerge LLC, a revolutionary communication and care coordination platform for senior care providers, announced today a new partnership with Lively, which offers activity-sharing products that help older adults live independently longer. Lively uses passive sensors applied to everyday household objects to measure daily activity levels and automatically notify family members, or other designated caregivers, when anything may be amiss. Under the terms of the partnership, activity data derived from Lively sensors will be combined with the clinical and social activity captured within Caremerge’s platform to correlate any physical health decline or frailty risks for caregiver intervention, improving care delivery and lowering hospital admissions.

 

The timely data will be available to stakeholders responsible for seniors’ health and wellness, including senior living communities, home healthcare, home care, family members, providers, managed and accountable care organizations, and more. For example, when medication is prescribed for a senior and managed via Caremerge, the Lively sensors will detect any changes in the older adult’s routine and correlate it with activities being tracked in Caremerge, resulting in actionable information that effectively complies with care directives.
